Student Check-ins & Befriending
Individual check-ins • Build trusted

relationships • Monitor well-being • Follow up

on previous concerns • Identify students

requiring additional support

Case Follow-up & Coordination
Liaise with teachers/YH/SC

• Parent communication • Document interventions •

Monitor support plans • Prepare cases

requiring further follow-up • Others ie. home

visits with FTs/Yr Heads if needed for

potential LTAs
