Job Description
1. To engage with identified students on a regular basis over a
prolonged duration and provide positive mentoring and/or counselling
support for students and their families.
2. To connect with identified students, build positive relationships and
contribute to their holistic development and positive school outcomes.
3. To plan and conduct student-centric interventions, including student
outreach, learning journeys, home visits and/or family engagement
sessions where necessary, customised to students’ needs and aligned
with the School’s efforts.
4. To plan and facilitate group-based programmes targeting students
with identified social, emotional, or behavioural needs, in consultation
with the School’s student support team.
5. To observe and identify key needs of students and to report such
cases to the Student Management Key Personnel and/or School
Leaders for further intervention and support.
6. To attend case review and case management meetings with relevant
school personnel, including the Student Welfare Officer (SWO), Special
Educational Needs Officers (SENOs), and school counsellors, to provide
feedback on students’ needs, programme experience, and concerns, so
that the School can better support the students.
7. To facilitate timely referrals of students and families to relevant
external agencies and community resources where additional support is
required beyond the School’s capacity.
8. To maintain accurate and up-to-date case records and
documentation, and to provide regular progress updates to the School’s
designated contact person.
