Speech Therapist (Geriatric & Community Care)

Job Description

Clinical Speech Therapy
Assess swallowing, speech, language, communication, and cognitive-communication needs.
Identify and manage dysphagia risks, including aspiration prevention and safe feeding.
Develop, implement, and review individualized Speech Therapy care plans.
Recommend diet textures, fluid consistencies, swallowing strategies, and communication supports.
Provide direct therapy, monitor outcomes, and respond to clinical queries.
Participate in case reviews and multidisciplinary meetings.
Documentation & Clinical Governance
Maintain accurate and timely documentation in designated electronic systems.
Ensure compliance with AHPC standards, clinical protocols, and infection control requirements.
Uphold professional ethics and client confidentiality.
Audit, Training & Quality Improvement
Conduct annual clinical audits and submit formal audit reports with recommendations.
Plan and deliver staff training and in-service sessions for nursing staff, care staff, and therapy assistants.
Support quality improvement initiatives and carry-over of therapy recommendations into daily care.
Service Continuity & Risk Management
Follow clinical safety protocols and escalate high-risk cases appropriately.
Support contingency arrangements and provide tele-consultations when required.
Workload (Indicative)
Monthly on-site clinical hours vary by facility.
Home Care & Home Hospice: up to 5 sessions/month, capped at 10 hours/month.
Additional annual hours for audits and training.
Qualifications & Registration
Degree in Speech & Language Therapy / Speech Pathology
Registered with Allied Health Professions Council (AHPC), Singapore
Valid practising status throughout contract period
Experience & Competencies
Experience in geriatric, nursing home, community, or home hospice care
Strong dysphagia assessment and management skills
Effective communication and documentation skills
Ability to work independently within multidisciplinary teams
Reporting Line
Functional reporting to Nursing Home / Home Care Management
Professional accountability to vendor organisation (if outsourced)
